The Chautauqua County Health Department is conducting a Community Health Assessment to collect data on the health status, needs, and challenges within the county. This initiative aims to inform the development of a Community Health Improvement Plan, which will guide resource allocation.
Residents are invited to participate in the Chautauqua County Community Health Survey. Participation is seen as a way for locals to contribute to enhancing community health. The survey, available until March 31, 2025, takes approximately 5-10 minutes to complete and ensures confidentiality of responses. Participants can opt to provide contact information separately for a chance to win one of five $50 Tops Gift Cards through a random drawing.
The survey can be accessed online at HealthyCHQ.com, with paper copies available at various community and medical sites throughout the county. Those needing assistance or additional paper copies can contact the county Health Department at 716-753-4491.
